Case Management Service

Coordinating the care and transport of medically fragile patients is an often complex task. There are many factors to consider to ensure safe transportation.

Our expert Case Management Team consist of Registered Nurses who have many years of clinical experience in critical care, emergency care, pediatrics, neonatal, respiratory care, cardiac care and trauma.

In addition to their clinical experience, all of our Case Managers have experience as Flight Nurse Coordinators, Flight Nurses, Ground Transport and Commercial Escort. Because of their extensive clinical experience and keen understanding of the transport of medically fragile patients, our Case Managers are able to provide comprehensive insight into the transport and repatriation process.

Our Case Management Service is designed to provide a comprehensive review of the patient's current medical state, determine the potential needs of the patient during the transport process and an ongoing evaluation of the patient's condition until the patient arrives at the desired destination. Elements of the Case Management Process include:

• Comprehensive review of the patient's medical condition
• Coordination with the sending hospital Case Management department and physician
• Evaluation of the patient's ability to be safely transported
• Recommendations of transport team makeup to best match the patient's needs
• Identification of special needs or considerations for transport of the medically fragile patient
• Daily review of the patient's status with updates to the client both in writing and via phone.
• Coordinate with receiving hospital medical team (Bed Finding Service Available).
