Medically fragile patients require medical transportation every day. Whether it's province to province, state to state or country to country, the logistics can be overwhelming. One of the most critical tasks in determining the timing of patient transport is a thorough Patient Assessment and medical review. The Case Management Team at Lifemedical Solutions Ltd have the resources to conduct full a Patient Assessment and provide you with timely feedback regarding the patient's status.

While this service is provided as a part of the Case Management Package, the ability to obtain a stand- alone Patient Assessment is also available. The key elements of this service include:

  • Gathering necessary biographical information regarding the patient
  • Reviewing the medical records of the patient's current condition and history of present illness
  • Interaction with the current hospital based care management team providing care to the patient
  • Review of the medical history, allergies and medications as well as current therapies and treatments
  • Review of special needs the patient may have during transport
  • Recommendation regarding patients suitability for medical transport
  • Recommendation for the best method of medical transport for this patient
  • Recommendation for the best team make up to transport the patient

If the situation arises that you will need additional Patient Assessments please consider using our Case Management Service. The Case Management Service provides a comprehensive daily review of the patient's needs until the patient is determined suitable for medical transport. With the Case Management Service, our dedicated Case Managers follow the patient from the time of initial consultation to completion of the transport disposition.
